You will be sent an email from the Enrollment office to confirm your registration forms were received. If you do not want to register online, you can pick up a Kindergarten registration packet at the Enrollment Office located at 310 West 24th Street. Contact the Enrollment Office at 698-8029 or ellie.schroeder@kearneypublic.org with questions. 1. Registration Form Please fill out the form online and hit the SUBMIT button. 2. Immunizations Please fill out the form online and hit the SUBMIT button. 3. Physical Exam This is required for students entering kindergarten. The form should be signed by a physician. 4. Health Questions Please fill out the form online and hit the SUBMIT button. 5. Certified Birth Certificate State law requires that all new students must provide a certified copy of the student's birth certificate. To obtain an application for a birth certificate go to the following site: http://www.hhs.state.ne.us/ced/bicert.htm 6. Proof of Residency A form such as a rent receipt, lease agreement, utility bill, or mortgage document is required for registration. The form must contain your name and current address. 7. FERPA Family Educational Rights and Privacy Act. Please fill out the form online and hit the SUBMIT button. 8. Home Language Survey Please print the home language survey and submit to 310 West 24th Street or email as an attachment to: ellie.schroeder@kearneypublic.org If you have trouble submitting forms online please open the form, complete, and save as an attachment.
